The process of apply Certificate of Origin issued by Thai Chamber of Commerce (known as C/O)

Document required REQUESTED FORM You will get this document (light green color not blue one - blue one is for C/F requested) form Thai Chamber Office. Fill in the information including your company code

CERTIFICATE OF ORIGIN (C/O) Form 110 Baht per 1 book which is contained 10-12 sets (7 sheets). Fill in the information base on your B/L which will related with your Invoice, Packing List and L/C. Signed and Stamp

INVOICE OR COMMERCIAL INVOICE 1 set with company stamp and signed

PACKING LIST 1 set with company stamp and signed

SEA FREIGHT - BILL OF LADING (B/L) = In case of B/L from MAERSK and EASTERN MARITIME 1 set of copy original B/L with MAERSK's stamp, company stamp and signed or 1 set of original non-negotiate with certify true copy, company stamp and signed including letter of identify that why we cannot get the copy of original copy for vessel = In case of Express B/L (Surrender) 1 set of non-negotiable with agent's stamp, company stamp and signed = Normal case 1 set of copy original B/L with company stamp and signed

AIR FREIGHT - AIR WAY BILL (AWB) 1 set of copy original AWB with company stamp and signed

EMS AIRWAY BILL OR COURIER AIRWAY BILL You have to type word "Consignor", "Consignee", "Notify Party", "Marks as per invoice no ", "Total Packing". Make a copy, stamp and signed

APPLICATION 1 day only if all of your documents are clear and corrected. Please do not forget that authorized signature have to be correct also.

REVISED If you need to revised your C/O, you have to Stamp and sign on the revised point Make a photo copy of C/O for Chamber reference Attached Invoice, Packing List and B/L 1 sets No need to use Requested Form, just contact to the officer and wait for a while then receive it back.
